1. Registration/Information Posting | 2. Stormwater Appearance
1a. Is the registration/permit/ID number clearly visible at site entrance?:
YES
1b. Is contact information posted with the registration information?:
YES
1c. Is the site plan posted at the entrance or available at the project trailer?:
NO
1d. Comments:
Plan not available at entrance - unknown whether it is available in trailer.
2a. Is sediment-laden stormwater retained on the site?:
NO
2b. Is sediment-laden stormwater prevented from entering a storm drain or waterbody?:
NO
2c. Comments:
There are still deficiencies on site that in my opinion will allow sediment/turbid water to leave the site. However, significant progress has been made at and around the site entrance.
3. Construction Entrance/Exits | 4. Perimeter Controls
3a. Is there enough stone, gravel, or stabilizing material to cover the entrance/exit?:
YES
3b. Are construction materials and equipment being stored away from the entrance/exit?:
YES
3c. Is the road free from dirt tracked from the site?:
NO
3d. Is the construction entrance/exit free from flowing dirty water?:
YES
3d. Comments:
There is still some dirt/staining of road from past events. No flowing water or sediment at this time although there is still a deficiency that will allow it to come around silt fence near entrance into drop inlet in event of significant rain event.
4a. Are perimeter controls present?:
YES
4b. Are the controls properly placed?:
NO
4d. Is water prevented from flowing over, under, or around controls?:
NO
4c. Are the controls properly trenched?:
NO
4e. Are the controls in good repair?:
YES
4f. Comments:
Control placement is such that they often do not provide enough ponding and have a good chance of failure. Also, in my professional opinion they are not properly trenched for the situation as a silt fence machine was not used. Only time - a large rain event will tell.
5. Storm Drain and Inlet Protection | 6. Soil Stabilzation and Cover
5a. Are storm drains and inlets protected and in good repair?:
NO
5b. Are storm drains and inlets free from sediment?:
NO
5c. Comments:
There are still inlet that are unprotected or inadequately protected.
6c. Are soil stockpiles properly seeded, covered with tarps, or surrounded by silt fence?:
NO
6f. Comments:
Soil is surrounded by silt fence but is on a rather steep sloped area - probably should be seeded or covered with plastic sheeting. Unsure about rills on internal parts of site.
7. Sediment Traps, Check Dams and Basins
7a. Are sediment traps, check dams, or basins installed?:
YES
7b. Do they have properly installed and operating components?:
NO
7c. Are the trap and basin slopes stabilized?:
NO
7d. Is the trap or basin retaining sediment?:
YES
7e. Comments:
Slopes of one of the ponds is still not properly stabilized. In my professional opinion the pond outlets should be covered with filter material or elevated to provide better treatment for turbidity removal.
8. Outlet Protection | 9. Encroachment on Waterbodies
9a. Is vegetation adjacent to waterbodies intact?:
NO
9b. Are waterbodies and adjacent buffers free of built structures, including basins and traps?:
YES
General Comments | Photos
10. General Comments:
There have been improvements - site entrance has been largely stabilized but still needs more stone and perhaps a little larger stone - 1st image, but there is an area where water will flow around a silt fence into an unprotected drop inlet - second image. A large sloped area (3rd image) on side of site near practice football field has been properly tracked up and down the hill and appears perhaps nearly ready for stabilization - hopefully this will take place before a significant rain event. Major remaining deficiencies involve soil storage areas and elevation of outlets in sediment detention ponds. More effort is needed to avoid offsite transport of highly turbid water upon next major rain event! 060110 - New Patrol/Comment - new images attached. GOOD: Large unprotected slope protected with ECBs as necessary for protection. NOT SO GOOD: Still tracking at entrance, still no freeboard and thus no treatment for suspended clay particles prior to discharge.
